3. Close PDF and Reopen PDF file.
Using the newest version of PDF program you just downloaded.

Fill out the form completely and digitally sign each area on the form.

4. Submit the Form: At the bottom of the PDF form click on to "CLICK HERE" button on the PDF to email the form back to SBM. If you encounter any issues, save the completed form to your computer and email the PDF to Superior Blinds MFG as an attachment PDF file to an email.
